Liquor License and Application Process
Gary L'Heureux, the Mayor of Midlothian, serves as the Liquor Commissioner for the Village. The Liquor Commissioner prioritizes the safety and welfare of the residents of Midlothian by centralizing control of the sales and services of alcoholic beverages within the Village. The Commissioner's primary responsibility in regulating the selling and serving of alcoholic beverages is providing liquor license applications, conducting site inspections, and enforcement of liquor ordinances and laws. Annual Liquor License Application
LIQUOR LICENSES ARE VALID FROM MAY 1 TO APRIL 30.

Completed applications must be submitted in-person Monday-Friday from 9am-5pm at the Police Department. All liquor license applications are reviewed and approved by the Liquor Commissioner.
Applicants should submit their new/renewal license application prior to April 30th to allow time for all necessary approvals before the issuance of the liquor license. Any renewal application received after the set due date will be assessed a $100 late penalty.
** Any incomplete applications will not be accepted at the time of submission and will be returned to applicant for completion.**
Application Checklist
- Completed Liquor License Application
- $250 non-refundable Application Fee (for first time applicants only)
- $40 Fingerprint/Background Check Fee (for each owner and/or manager)
- Any renewal application received after the set due date will be assessed a $100 late penalty.
- Photocopy of Drivers License or State ID
- Copy of Dram Shop Insurance (naming the Village of Midlothian as additional insured)
- Midlothian Business License
- Copy of Lease or Ownership Deed for Business Property
- Basset
- Liquor License Classification Payment and Applicable Fees (upon approval of application by the Liquor Commissioner) Shall be Made by Check or Money Order made payable to Village of Midlothian
