Do I need a Village sticker?: Yes, Stickers can be purchased online or via mail. Vehicle stickers will not be sold at Village Hall. Applications will be mailed to residents at the end of April and stickers will be available for purchase the first business day in May. Stickers can be purchased through June 30. A Midlothian vehicle sticker must be obtained and displayed within thirty (30) days of purchase of said vehicle by a Midlothian resident or within thirty (30) days after a new resident moves into the village of Midlothian. If a new or additional vehicle is purchased, you will need to bring your registration information to the Village within 30 days. 1. Who can I contact about have a question/complaint about my garbage pickup, questions regarding bulk items or need additional garbage cans?: You can call Republic Services at (708)-385-8252 and speak to someone about your garbage service.

3. Does my Pet need a Pet tag?: Yes, All dogs and cats within the Village of Midlothian are required to have a pet tag. There is a combined total limit of (3) dogs or cats per household and a rabies tag for each pet is required for purchase. Applications will be mailed to residents at the end of April. If you are new to the Village or need to add/remove a current pet, a rabies certificate will be needed to register your pet and their shots must be up to date receive a tag.

4. Do I need a permit to have a Garage Sale?: Yes, Any person who conducts a garage sale within the Village of Midlothian must first obtain a garage sale permit. Come to Village Hall before your sale to request a permit. The cost of a garage sale permit is $2 per event. 5. How can I get a parking spot for the West Commuter Metra lot?: For more information on the West Commuter lot or to get on the waiting list, please call Village Hall during normal business hours at (708) 389-0200 option 5.

6. Is DMV open & what is their phone number?: To contact the DMV, call the Secretary of State at 312-793-1010.

7. Who do I report a pothole to on Pulaski?: Contact the IL. Dept. of Transportation at 847-705-4610.

8. Who do I contact if I need help paying my bill and need tax information?: Contact Bremen Township at 708-333-9530.

10. Who takes care of Dead wildlife?: To report dead wildlife, contact Cook County Animal Control at 708-974-6140.

11. Who do I contact for tickets or court info, or Birth, Death & Marriage Certificates?: For information about tickets or court info, or Birth, Death & Marriage Certificates, call the District 6 Markham courthouse at 708-232-4551.

12. Can I rent a park or Raday Lodge?: The Raday Lodge is no longer used for rentals.

13. Can I book Midlothian Meadows for a party?: To book Midlothian Meadows for a party, contact the Forest Preserve at 708-447-0090.

14. Can I rent the Gazebo on the Village green?: To rent the Gazebo at the Village Green, contact Maria Kolacki at 708-389-0200 option 6.
